What is Lipofilling?
Lipofilling is an advanced surgery technique. It uses the patient’s own fat obtained by liposuction to fill in hollows and/ or create volume. The patient’s own fat is reinjected, creating an autologous graft of adipocyte cells (fat graft). This technique is also called: lipostructure, lipomodelling, lipofilling, fat injection, autologous fat transfer, adipose graft or gluteal augmentation and lift. All of these are synonyms.
As far as buttocks are concerned, it is called Brazilian Butt Lift (BBL), butt liposculpture, buttock augmentation, buttock gluteal augmentation, buttock lipofilling, buttock contouring…
What is the benefit of lipofilling in buttock augmentation?
Lipofilling has first been used in breast reconstruction called liposculpture, lipostructure, lipofilling or lipomodelage. With this experience and efficiency, lipofilling has been used in other parts of the body (face, limbs, hands …) including esthetic buttock augmentation.
Fat transfer in buttocks is recognized as being reliable when performed by a board-certified plastic surgeon with experience in butt lipofilling.
What are the indications for Buttock Lipofilling?
Buttock Lipofilling is for patients who need:
- Buttock augmentation without implants
- Buttock augmentation without scars
- A buttock lift without scars
- To improve the shape of sagging and flat buttocks
- Liposuction to reshape their figure to create a slimmer silhouette and achieve an “S” profile body shape.
To undergo a lipofilling, the patient must have enough fat tissue stock to be harvested for use in one or two lipofilling sessions.
Very slim patients are not good candidates for buttock lipofilling.
PRINCIPLES OF A BUTTOCK LIFT WITHOUT SCARS
What are the major benefits of a fat transfer to buttocks?
- It uses your body’s own fat to augment the size of your butt in a natural way and with a natural outcome.
- Once the graft has settled, the result is permanent and the size of the buttocks will vary like any other fat in your body.
- The result is natural, the skin is soft and supple.
- There is no scar.
- As lipofilling uses the patient’s own fat, it is an autologous procedure, 100% natural.
- As there is no implant, there is no risk of scar, capsular contracture, rejection or rupture…
- Inharmonious areas of the body are treated at the same time (as the fat is harvested).

BEFORE BUTTOCKS LIPOFILLING:
- Two preoperative consultations with a board-certified plastic surgeon will be conducted as well as a consultation with an anesthetist.
- Before and after comparison photographs will be taken.
- You will be prescribed a compression garment for buttocks lipofilling.
- Preoperative instructions are as follow:
- Quit smoking at least one month before the surgery and up to one month after the surgery to avoid any complication or necrosis risk.
- Discontinue any anti-platelet or blood-thinning medication 15 days before the surgery.
- Stop birth control pill 1 month before the surgery.
BUTTOCKS LIPOFILLING: THE PROCEDURE;
- It is performed under general anesthesia, most of the time, depending on the number of areas where the fat is to be removed.
- The procedure lasts about 2 hours depending on the volume of the fat injected.
- Dr Vincent Nguyen performs liposuction and lipofilling on both sides himself.
It will take up to 6 months to see the final outcome. As not all the fat survives the transplant, a second session is sometimes needed to achieve the expected result.
AFTER BUTTOCKS AUGMENTATION WITH LIPOFILLING:
- As buttocks lipomodelling is not a painful procedure, only light analgesics will be prescribed.
- Patients may go home the following day or the same day.
- Swelling and bruising occur on the buttocks and on the collection sites the following days.
- No nursing care will be needed.
- You may shower 24 hours after the surgery.
- A compression garment will be worn 4 weeks, night and day, to help remodel tissues.
- It is advised to avoid direct sitting for a week to 10 days after buttocks lipofilling but patients may sit on a butt pillow to avoid pressure.
- Most patients may go back to work after 10 to 15 days depending on the nature of their job.
- Physical activities may be resumed after 6 weeks.
- Follow-up consultations will be planned with Dr Nguyen (1 week, 1 month, 3 months, 6 months and a year after the procedure).

- As it is an esthetic procedure there is no coverage or reimbursement by Social Security or Health Insurance Company.

Is Buttocks Lipofilling permanent?
All the fat that survives the transplant after 3 months will be permanent but it will be affected by weight gain or weight loss as in any other parts of your body.
History of lipofilling
The method of using fat is an established technique in plastic surgery that has been developed and improved over the years. For 30 years the techniques of harvesting, purifying and reinjecting fat have considerably improved. It is a simple, reproducible and reliable technique. Dr Coleman, an American plastic surgeon and pioneer of the procedure, has given his name to a lipofilling technique called “the Coleman technique”.
What are the drawbacks to buttocks augmentation with lipofilling?
- The volume of injected fat depends on the fat tissue stock. Some very slim patients don’t have enough excess fat to make a successful buttock lipofilling. Dr Vincent Nguyen will evaluate your needs, the quantity of fat available and will discuss your options.
- As the procedure involves autologous fat graft, a certain amount of fat is absorbed by the body. After 3 months, up to half the fat injected may have been absorbed.
- A second lipofilling procedure may be performed if the patient wants more volume and curves.
- The final result is not entirely predictable.
What are the alternatives to buttock augmentation with lipostructure?
- If a buttock lipofilling is not an option for you, you may consider implants as an alternative to fat transfer if you are self-conscious of your flat buttocks.
- Both techniques (butt implants and lipofilling) may be combined. (see information sheet).
What other procedures may be combined with buttocks lipofilling?
- It may be combined with a buttock lift if the skin of your buttocks is loose and sagging. An incision line will be made under the buttock cheeks or at the top of the buttocks.

Are there other indications for buttocks lipofilling?
- It can be performed in case of congenital defects or deformities.
